FORMS FOR ANY YOUTH MINISTRY EVENT (anything outside of regular youth meetings)
You must have a completed and signed diocesan 1. Field Trip Consent Form (there are two different forms, one for an overnight event and one for a non-overnight event) and a 2. Medical Release Form for each teen participating in each event you offer. If you plan to take pictures or video at your event, you must also have a 3. Communication/Media Release Form on file.

REGULAR FORMS AS PART OF ANNUAL REGISTRATION
As a best practice, the medical and media release forms should be completed at the beginning of the ministry year with each teen’s ministry registration/information form, and can be copied by the youth minister for each event during that ministry year.

CHAPERONE VERIFICATION FORMS
All extended events off of church property (retreats, mission trips, camps, conferences, etc.) - especially if outside of the diocese - require the diocesan Chaperone Verification Form. It should be emailed to Michael Craig in the diocesan Safe Environment Office at mjc@dosp.org.
